Heart Rate Response In Maximal Exercise Is Related With Coronary Heart Disease
PURPOSE: Coronary heart disease (CHD) has been identified as the leading cause of death in sedentary males and the correlation between heart rate (HR) response and CHD is unknown. In this study, we aimed to investigate the efficacy of HR response as a predictor of CHD in physical-inactivity men. METHODS: Physical-inactivity patients with or without CHD were included and they performed cardiopulmonary exercise testing (CPET). Resting HR before CPET, HRpeak during CPET, and post-exercise resting HR (1 min and 2 min) were measured, other HR response parameters were calculated. The relationship of HR response and CHD was analyzed in different severity. RESULTS: 92 male patients completed CPET. heart rate recovery at 1 min (HRR 1 min), heart rate recovery at 2 min (HRR 2 min), HR reserve and HRpeak-rest were significantly different between CHD and non CHD (P < .05), the odds ratio separately was 0.96 (95% CI, 0.93-0.98) in HRR 1 min; 0.95 (95% CI, 0.92-0.98) in HRR 2 min; 0.97 (95% CI, 0.94-0.99) in HR reserve; 0.96 (95% CI, 0.93-0.99) in HRpeak-rest. And the optimal cutoffs for a diagnosis of CHD were HRR 1 min <104.5 (sensitivity, 76.9%; specificity, 59%; area under the curve [AUC], 0.729 [0.625-0.833]), HRR 2 min <95.5 (sensitivity, 69.2%; specificity, 64.1%; AUC, 0.724 [0.621-0.826]), HR reserve <57.35 (sensitivity, 86.5%; specificity, 48.7%; AUC, 0.696 [0.587-0.805]), HRpeak-rest < 51.00 (sensitivity, 86.5%; specificity, 43.6%; AUC, 0.681 [0.570-0.793]). There was no significant difference in all HR parameters between different severity of CHD. CONCLUSIONS: Both HR recovery parameters and HR reserve parameters were linked to an increased risk of CHD, and HRR 1 min had the highest predictive accuracy.the Fundamental Research Funds for the Central Universities (Key Laboratory of Exercise Rehabilitation Science of the Ministry of Education) at Beijing Sport University
