Improving the organization of dispensary observation of patients with chronic dermatosis.

One of the most important sections of a doctor’s work is the organization of dispensary observation of patients with chronic diseases. To ensure the necessary volume and quality of dispensary observation for patients with chronic dermatoses, an integrated approach is required. The purpose of our study was to develop and evaluate the effectiveness of measures to improve the organization of dispensary observation of patients with psoriasis. Materials and methods. To improve dispensary observation of patients with psoriasis, we have developed a set of measures. The effectiveness of the implemented measures was assessed in two groups of patients with psoriasis living in rural areas. The first group – patients before the implementation of the proposed measures (145 patients), the second group – after the implementation of the proposed measures (168 patients). In both groups, patients’ adherence to treatment, the quality of clinical supervision and the quality of life of patients were assessed. The opinion of doctors about the results of the measures implemented in their work was studied. Results. The introduction of measures to improve dispensary observation of patients with psoriasis in the work of medical organizations of the Republic of Bashkortostan has shown their effectiveness. The number of patients with improvement in the second group was 78.6±3.2%, and in the first group it was one third less – 51.0±4.2% (p<0.001), and the number of patients without changes in condition, on the contrary, was greater in the first group was 37.9±4.0% and 10.3±2.5%, than in the second group – 15.5±2.8% and 5.4±1.7% (p<0.001). There was an increase in the coverage of patients with dispensary observation by 61.7%, a decrease in the number of patients with relapses of the disease by 36.5%, a decrease in the number of hospitalizations for exacerbation of the disease by 46.1%, an increase by 34.2%. with coverage of anti-relapse outpatient treatment, the number of days of temporary disability decreased by 42.4%, compliance and the level of quality of life increased. According to the surveyed doctors, the implemented measures had a positive impact on the work with patients of the dispensary group. Conclusion. Thus, the set of proposed measures to improve dispensary observation of patients with psoriasis has shown its effectiveness and can be recommended for further implementation in medical organizations providing medical care to patients with chronic dermatoses.
