J026 Physiotherapeutic interventions in Huntington’s disease- real-world data from ENROLL-HD

Background
Since there are no disease-modifying therapies in Huntington’s disease (HD), physiotherapy (PT) is one important aspect of interprofessional disease management. A meta-analysis showed positive short-term effects of physiotherapeutic interventions on general endurance, lower limb strength and cognitive performance. Until date, there is still a knowledge gab regarding the real-life implementation of PT in HD.
Aims
This project aims to evaluate to what extend people affected by HD (diagnostic confidence level 4) make use of PT in a real-world cohort. We therefore analyze the periodic data set five of the global observational study Enroll-HD. Furthermore, an influence of PT on the disease progression shell be evaluated using longitudinal data.
Methods
Baseline ENROLL-HD data was analyzed regarding affected HD patients with different PT-intensity levels (daily, weekly, none). Group comparisons were made to investigate motor (total motor score (TMS)), functional (total function capacity (TFC), independence scale (IS)), cognitive (mini mental state examination (MMSE)) and psychiatric (problem behavior assessment (PBA)) parameters between groups.
Results
In total, N= 3245 manifest HD patients were analyzed whereby 13% received PT in different intensity levels compared to 87% without PT. The CAG repeat expansion was similar in all groups (p=0,776). No differences were depicted for age, HD diagnosis and manifestation-age. Participants receiving PT were more affected in motoric, functional, and cognitive performances (TMS, TFC, IS (p<0,05), MMSE (p<0,001)).
Conclusion
Physiotherapeutic interventions are mainly considered when motoric deficits are present and less as a preventive therapy in symptomatic HD patients. Further analyses will be conducted to analyze matched patients with regard to CAP-scores to depict longitudinal therapeutic effects of PT.
