EP11.21: Management of ovarian masses in pregnancy

Ovarian masses are one of the most common complications during pregnancy with an incidence of about 1-5.3%. Most of them are functional or benign, such as corpus luteum and theca-lutein cysts. Approximately 5% of ovarian masses in pregnancy are malignant. Although US and MRI have a high sensitivity and specificity, when the diagnosis remains uncertain, histology confirmation is required. Complications are similar to the non-pregnant population and include torsion, rupture or bleeding, progression in case of malignancy, but also premature labour. When the probability of malignancy is high or if there is a high risk of developing complications surgery is indicated. Here we present a 40-year-old woman with twin pregnancy. In 13th gestational week (GW) she presented with nausea and abdominal pain. US examination detected bilateral multilocular cystic ovarian masses with thin septa and moderate vascularization (RI 0,46), filling the pelvis and abdomen up to the left hemidiaphragm. Previous US examination in 9th GW was normal. Due to the inconclusive diagnosis regarding malignancy and high risk of complications, laparotomy and bilateral ovarian cystectomy was performed. Histopathological finding reported bilateral hemorrhagic corpus luteum cysts. Pregnancy was successfully carried to term. Early US detection and characterization of ovarian masses in pregnancy is crucial for further diagnostics and appropriate management, since it can affect both mother and child morbidity and mortality.
