EP10.22: Maternal propranolol treatment for fetal pleural effusion: a case report

A 28-year-old multigravida presented at 36 weeks gestation with a large unilateral fetal pleural effusion and mild polyhydramnios. No other anomalies were seen at diagnosis or on previous ultrasound examinations. The most likely diagnosis was an isolated chylothorax. Management options included delivery, fetal thoracocentesis or medical management. After counselling, the parents opted for maternal therapy with 20mg oral propranolol 3x/d. The pleural effusion and mild polyhydramnios resolved in three days. At 37 weeks and 4 days the mother went into spontaneous labour and delivered a 4050g neonate vaginally without complication. The neonate was discharged home after 3 days and is well without recurrence of an effusion.
