EP23.59: IOTA simple rules to diagnose the benign and malignant value of ≤ 6cm masses in the adnexal area

IOTA simple rules to diagnose the benign and malignant value of ≤ 6cm masses in the adnexal area. From January 2016 to December 2022, 217 consecutive patients (a total of 232 small lumps) who were treated in the Department of Obstetrics and Gynecology Ultrasound of the First Affiliated Hospital of Xinjiang Medical University with an average of ≤ 6cm of the three vertical diameters of small masses, the ovarian-accessory report and data system (O-RADS) was used to standardise the description of small masses in the adnexal area, followed the principles of synchronization and blinding, and use the IOTA simple rule to judge its benign and malignant tumour, and then the results were compared with the gold standard of pathology and the results of long-term follow-up to evaluate the accuracy of IOTA simple rule in diagnosing the ≤ 6cm adnexal masses. Among the 232 small masses, 176 were benign masses, 50 were malignant tumours, and 6 were borderline tumours. Among the 50 malignant tumours, 5 were missed as benign masses; 14 were indeterminate lumps. Among the 176 benign masses, 5 were misdiagnosed as malignant tumours and 35 were indeterminate masses. The sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, positive predictive value, negative predictive value and coincidence rate of the≤ 6cm benign and malignant masses in the adnexal region were 91.0%, 77.2%, 4.00, 0.11, 56.0%, 96.4% and 74.6%, respectively. The area under the ROC curve of IOTA's simple rule for the diagnosis of benign and malignant small masses in the adnexal region was 0.842, with a 95% confidence interval of (0.784, 0.899). The application of IOTA simple rule to distinguish the ≤ 6cm benign and malignant masses in the adnexal area has a higher diagnostic value, but the optimization of M4 and M5 malignant features can be tried to improve the diagnostic efficiency.
