EP23.13: Ultrasound characteristics of borderline ovarian tumours selected by ADNEX model

To get closer view on ultrasound characteristics of borderline ovarian tumours (BOTs) selected by ADNEX model in a routine, specialist scan setting. Preoperative distinguish of borderline suspected ovarian lesions is usually made by subjective assessment especially when experienced examiner is available. Ultrasound markers such vascularised, multiple papillary projections and honeycomb nodules are often associated with BOTs but the presence of these not equals with BOT immediately. The aim of this study was to check necessity of ADNEX support with some patterns to discriminate BOTs more precisely? In tertiary university hospital's specialised ultrasound centre 613 patients were scanned during a 6-year interval. Detailed preoperative ultrasound scans were made using IOTA terms and definitions by an experienced specialist in gynecological ultrasonography. ADNEX calculation was matched with histological findings in those patients who underwent surgical intervention. 613 patients (10-93yrs, avg: 47.08yrs) have been scanned. Of those with increased ADNEX score those with highest relative risk increase for BOT were selected (N = 94), 59.57% (N = 56) of these tumours underwent surgical removal. Histological findings proved the presence of BOT in 12.5% (N = 7) of the removed tumours. Of the BOT specific ultrasound markers multiple papillary projections were present in 85.71% (N = 6) of the cases while honeycomb nodule was visible in one case (14.29%). At least moderate blood flow (colour score 2) was present in all cases, majority was colour score 3 and 4 (N = 6, 85.71%). Of the remaining surgically removed tumours (N = 49) 23 turned to be malignant (46.94%) and 26 (53.06%) was a benign lesion of which 15 (57.69%) contained papillary solid parts but only 4 (26.67%) of these had visible vasculature. ADNEX model is a reliable tool for preoperative selection and discrimination of BOTs from invasive malignant tumours and atypical benign lesions as well. Regarding discrimination from benign lesion some other markers such vascular patterns should be considered to elevate the effectivity.

Abstract

Histological findings proved the presence of BOT in 12.5% (N = 7) of the removed tumours. Of the BOT specific ultrasound markers multiple papillary projections were present in 85.71% (N = 6) of the cases while honeycomb nodule was visible in one case (14.29%). ADNEX model is a reliable tool for preoperative selection and discrimination of BOTs from invasive malignant tumours and atypical benign lesions as well.
