EP17.52: Comparison of non‐invasive maternal hemodynamics at 13 to 16 weeks between pregnancies with low and high risk of preterm pre‐eclampsia

This study aims to compare non-invasive maternal hemodynamics between low-risk and high-risk preterm pre-eclampsia (p-PE) cases, as determined by the FMF algorithm, during the 13 to 16-week period. Enrolled subjects between September 2023 and February 2024 underwent p-PE risk assessment at 11-14 weeks using the FMF algorithm. Exposed and non-exposed subjects were categorised based on a risk > 1:100 and < 1:500 (with normal UtA), respectively. Hemodynamic evaluations were performed between 13 and 16 weeks of pregnancy. UtA pulsatility index (PI) measurements were obtained for analysis. Non-invasive maternal hemodynamics were assessed using bioreactance, considering cardiac output (CO), peripheral vascular resistance (PVR), systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), and heart rate (HR) for analysis. Statistical analyses involved Mann-Whitney U test. The FMF risk of exposed (n = 20) and non-exposed (n = 38) group was 1:54 (1:39 – 1:82) and 1:977 (1:688 – 2,847); p < 0.0001. At 13 to 16 weeks assessment, the high-risk group showed a higher BMI than low-risk group (26.8 [24.2 – 30.4] kg/m2 vs. 23.3 [21.6 – 24.5] kg/m2; p = 0.0002, respectively). Mean UtA was higher in the high-risk group compared to low-risk group (1.56 [1.02 – 1.83] vs. 1.20 [0.96 – 1.52]. As expected, the high-risk group had a higher SBP (p = 0.003), DBP (p < 0.0001) and MAP (p = 0.0001) than low-risk group. Despite that CO showed no difference between groups (p = 0.68), the PVR was higher in high-risk pregnancies (1,257 [1,127 – 1500] dynes/sec/cm-5 vs. 1084 [923 – 1,200] dynes/sec/cm-5, respectively; p = 0.01). Body surface area was similar between groups (p = 0.052). The first trimester FMF algorithm effectively identifies a high-risk group demonstrating elevated PVR during the early second trimester.
