EP17.50: Cervical elastography and the risk of preterm birth in pregnant women with a cervical length of 2.5–2.9 cm in midtrimester

To investigate the differences in cervical elastographic parameters in pregnant women with or without preterm birth (PTB) who were diagnosed with a cervical length of 2.5–2.9 cm in midtrimester. This was a secondary analysis of a multicentre prospective observational study by the Korean Research Group of Cervical Elastography involving 7 referral hospitals. Singleton pregnancies with a cervical length of 2.5-2.9 cm diagnosed at 18-24 weeks of gestation were enrolled. Cervical elastographic parameters were measured using E-Cervix™ (WS80A, Samsung Medison, Seoul, Republic of Korea). Women with progesterone use, and cerclage before or after cervical elastography were excluded. PTB was defined as delivery < 37 weeks of gestation. Cervical length and elastographic parameters were compared between term and preterm delivery groups. The area under the curve (AUC) analyses were performed. A total of 105 patients were included in the analysis. Eight women (7.6%) had spontaneous PTB. The mean gestational age at cervical elastography measurements was 21.1 weeks. Mean gestational age at delivery was 38.9 and 35.6 weeks in term and preterm birth groups, respectively. Although cervical length was not significantly different between the groups, EOS strain, ECI, strain mean, and hardness ratio were different between the two groups. (see table 1). The AUCs for EOS strain, ECI, strain mean, and hardness ratio were 0.8067, 0.7229, 0.7697, and 0.7912, respectively. There were differences in cervical elastographic parameters using E-Cervix™ in singleton pregnancies with and without PTB complicating with a cervical length of 2.5-2.9 cm in midtrimester, which may help in predicting PTB. Abstract
