EP09.21: Quantitation of right atrial unction in fetuses with moderate to severe tricuspid regurgitation using two‐dimensional speckle tracking echocardiography

To investigate the application value of 2D-STE in the measurement of global longitudinal peak strain of right atrium (PRAGLS) in normal fetuses and fetuses with moderate and severe tricuspid regurgitation (MSTR). Twenty fetuses with MSTR were selected with GA of 17~38(27.7 ± 5.2) wks, and 100 normal singleton fetuses as controls, with GA of 26 (23,29) wks. Standard basal or apical four-chamber clips were obtained and inputted into TOMTEC-ARENA offline cardiac analysis software. The fetal PRAGLS 2 groups were acquired and FHR, AO diameter, PA diameter, PA/AO ratio, RA end-systolic length(RAESL), RA end-systolic diameter(RAESD), RA end-systolic area (RAESA), RV end-diastolic diameter (RVEDD) and TAPSE, the peak TR velocity and PG were measured in the case group. The differences of the fetal PRAGLS and other parameters between 2 groups were compared and analysed. The correlation between fetal PRAGLS and GA and routine measurements was compared between 2 groups. There were significant differences in PRAGLS between 2 groups (difference 13.93, 95% CI: 11.55-116.32, t = 11.55, P < 0.01). PRAGLS and GA in the controls were negatively and moderately correlated (r = -0.47, P < 0.01). PRAGLS were significantly correlated with BPD, FL, AO, PA, RAESL, RAESD, RAESA and RVEDD (P < 0.05) in controls, but not with maternal age, FHR, PA/AO ratio and TAPSE (P > 0.05).There was no significant correlation between PRAGLS and maternal age, GA, FHR, BPD, FL, AO, PA, PA/AO ratio, RAESL, RAESD, RAESA, RVEDD, TAPSE, TR peak velocity and PG in MSTR group (P > 0.05). The inter- and intra-measurement ICC of PRAGLS values were 0.87 and 0.79, respectively. 2D-STE has good feasibility and reproducibility in the assessment of fetal atrial function, fetuses with MSTR shown decreased PRAGLS, implying right atrial reservoir function impaired in fetuses with MSTR.
