EP25.18: Ultrasound screening of Müllerian anomalies

The study aimed to assess prevalence of Müllerian anomalies in reproductive-aged women and the effectiveness of 2D and 3D ultrasound in screening and diagnosing these anomalies. A retrospective cohort study was conducted from December 2015 to December 2023 on all reproductive-aged women who had a 2D and 3D ultrasound. This study determined the incidence, sensitivity, and specificity of screening with 2D and 3D ultrasound modalities. Z-test was used to compare the effectiveness of each modality. A total of 11,284 transvaginal ultrasounds revealed 173 Müllerian anomalies (1.53%). In this study, 2D ultrasound identified 118 cases of Müllerian malformations (68.2%), with 92 confirmed by 3D ultrasound. Meanwhile, 3D ultrasound accurately diagnosed all 173 cases(100%), effectively clarifying all cases that were initially suspected by 2D ultrasound. Within the cohort identified by 3D ultrasound, the malformations were categorised as follows: arcuate uterus (58.4%), septate uterus (18.5%), bicornuate uterus (14.4%), didelphys uterus (5.2%), and unicornuate uterus (3.5%). Sensitivity of 2D ultrasound was 53.1%, with specificity of 99.5%, while 3D ultrasound achieved 100% accuracy (p < 0.001), substantially improving detection rates. 3D ultrasound plays a crucial role in accurately identifying Müllerian anomalies, ensuring thorough and effective prenatal care. Transvaginal 3D ultrasound is essential for detecting Müllerian malformations in women planning pregnancy, enabling preconceptional intervention, particularly beneficial for conditions like septate uterus, leading to improved pregnancy outcomes. Its routine utilization enhances detection and management, supporting informed reproductive planning.
