EP09.38: Placental pathology in pregnancies complicated by right heart obstructive lesions

Research links placental vascular malperfusion (VM) to coronary heart disease (CHD), stressing the need for early recognition in diagnosis and management. Our study focuses on placental characteristics in pregnancies with right heart obstructive congenital lesions (RHO), correlating with morphological and ultrasound anomalies. From March 2021 to October 2023, 112 CHD-affected fetal placentas were analysed. The obstetric ultrasound data were collected retrospectively and expressed according to the centile system recognised in the literature. We then categorised the data according the diagnosis into RHO+ and RHO-. A higher BW/PW ratio (RHO+:6.2 ± 2.0; RHO-:5.9 ± 2.2, p = 0.36) in RHO+ compared to RHO- was due to lower placental weight in centiles (RHO+:58.1 ± 40.7;RHO-:62.1 ± 37.0, p = 0.94). As shown in table 1, RHO+ cases displayed exclusive fetal VM, with notably higher avascular villi and fetal inflammatory infiltrate. DV PI is significantly increased in RHO+ group, graphs highlight key morphological and obstetric functional findings. Our data show that RHO+ are the only placentas with signs of VM on the fetal side in the presence of a markedly elevated DV PI. This is certainly an association that deserves further investigation, within the context of a pathophysiology unique to RHO+, clearly distinct from others. RHO+ (n = 30) n (%) RHO- (n = 82) n (%) Please note: The publisher is not responsible for the content or functionality of any supporting information supplied by the authors.
