EP16.12: Accuracy of antenatal ultrasound evaluation of the placental pathologies using superb microvascular imaging

SMI (superb microvascular imaging, Canon Medical Systems, Japan) is a Doppler method that enables the visualization of low velocity blood flow. In this study, we aimed to clarify whether SMI can detect prenatal pathological findings in pathologic placentas. Prospective diagnostic observational study was performed. Pregnant women who were admitted to our perinatal centre for perinatal management were enrolled. Ultrasound examinations to classify placental pathologic conditions using SMI were performed before delivery. Ultrasound findings were classified into following four conditions including infarction, avascular villi, congestion, and chorangiosis (see figure 1). After delivery, the placental tissue was clipped for microscopic examination, at the location of the placenta obtained ultrasound findings. The accuracy of prenatal ultrasound detection of placental pathologic findings was determined in women who were admitted due to fetal growth restriction (FGR), pre-eclampsia, and other indications. Thirty-four pregnant women were analysed. The accuracy of prenatal ultrasound detection of placental pathologic findings using ROC curves are compared. In cases with FGR, the most accurate diagnostic finding was placental infarction (PPV, 100%; sensitivity, 88.9%; AUC 0.945), avascular villi (AUC, 0.785), congestion (AUC 0.537) and chorangiosis (AUC 0.655). In cases with pre-eclampsia, infarction (AUC 0.700), avascular villi (AUC 0.784), congestion (AUC 0.530), and chorangiosis (AUC 0.520). It became clear that SMI can accurately detect placental pathologic findings, such as placental infarction and avascular villi. This modality may improve the perinatal management in cases of placental dysfunction. Abstract
