EP08.32: The role of fluximetry in fetal outcome
To compare the role of cerebroplacental ratio in the cutoff value for predicting the fetal outcome in pregnancies with fetal hypotrophy. This is a retrospective study done in University Hospital “Queen Geraldine” in pregnancies with fetal hypotrophy from January 2023 to January 2024. Pulsatility index in the medial cerebri artery and the umbilical artery are converted in cerebroplacental ratio. Efficacy of cerebroplacental ratio in fetal outcome is referred to the graph of this ratio and 1.08 is taken as a reference value. In the negative fetal outcome are included delivery with Caesarean section, Apgar in the fifth minute, hospitalisation in the neonatal intensive care, fetal complications (neonatal convulsions, fetal intraventricular hemorrhage, mechanical ventilation, respirator distress). In 90 fetuses with hypotrophy only 36 of them had an abnormal cerebroplacental ratio.18 of 144 of fetuses with a normal cerebroplacental ratio(12.5%) had a negative fetal outcome compared with 34 of 36 of fetuses with an abnormal cerebroplacental ratio(94.4%). This shows the importance of cerebroplacental ratio with sensitivity, specificity, positive and negative predictive value 68%, 98%, 94%, 88% each. The best result to predict fetal outcome is taken using cerebroplacental ratio(90% accuracy) compared to the umbilical artery(83%) and cerebri media artery(78.8%). An abnormal cerebroplacental ratio is associated with a not good perinatal outcome in fetuses with fetal hypotrophy. Our study confirms that cerebroplacental ratio is a better predictor of fetal outcome compared with umbilical artery and cerebri media artery studied separately.
