EP16.29: Comparative analysis of ultrasound and MRI in diagnosis of placenta accreta spectrum
Placenta Accreta Spectrum is a persistent challenge for obstetrics ultrasound especially Placenta Accreta. Recently, with the addition of MRI in making diagnosis of invasive placenta, there are conflicting data regarding added and separate values of Ultrasound and MRI. We aim to assess both individual and combined ability of Ultrasound and MRI to diagnose and see the sensitivity and specificity in detecting Placenta Accreta Spectrum in suspected cases. We assessed 58 cases of suspected Placenta Accreta Spectrum in the third trimester during a period of one year. All cases underwent Ultrasound as well as MRI. Diagnosis was confirmed during surgery and histopathology. A total of 43 cases were diagnosed with Placenta Accreta Spectrum. Our study showed Ultrasound to be more sensitive and specific as compared to MRI (Sensitivity of 0.86 and specificity of 0.5 in Ultrasound vs. sensitivity of 0.76 and specificity of 0.4 in MRI). However, we noticed that structured MRI reporting with details of involvement in cases of Placenta Percreta showed similar level of Ultrasound sensitivity and specificity. Ultrasound is superior to MRI in analysis and diagnosis of Placenta Accreta Spectrum. However, MRI with structured reporting reaches similar sensitivity and specificity in cases of Placenta Percreta.
