EP05.54: Intraventricular hemorrhage grade III
We report a case 36 week 3 days gestation fetus with intraventricular hemorrhage grade III. First and second trimester screening was also in our unit and reported as normal. But at the 36 week 3 days screening was revealed severe ventriculomegaly, dilated anterior horns and third ventricle, hyperechogenic ventricular walls, asymmetric mass with an external echogenic rim and an internal sonolucent core within the ventricles. Which shows that the over time, as the blood clot ages, lysis occurs. Cardiomegaly was also revealed. Parents were informed about grading and neurologic outcomes. Delivery was induced. 2 month brain neurosonography was done in our unit and hydrocephaly was diagnosed. Now the baby is 8 months old and neurological status is normal. Fetal intracranial hemorrhage is quite rare. It is generally diagnosed in the late second trimester as an asymmetric echogenic mass within the ventricles, mostly associated with some degree of ventriculomegaly. Causes include drug use (warfarin, cocaine) alloimmune thrombocytopenia, coagulation disorders or trauma. There are three types of intracranial hemorrhages – intraventricular, intraparenchymal and subdural. Intraventricular is the most common and categorised in severity on a four-point scale (I, II, III, IV). The outcome is usually poor for fetuses with high grade.
