EP22.13: Application of transrectal dual‐plane contrast‐enhanced ultrasound in the diagnosis of vaginal neoplasms: report of three cases

To study the value of transrectal dual-planar probe ultrasound in the examination of normal vaginal anatomy and CEUS in the diagnosis of vaginal lesions. An appropriate amount of sterile coupling agent was squeezed into the vagina as a sound window, and a dual-planar probe was placed into the rectum to analyse the normal structure of the vagina. The lesion's size, boundary, margin, shape, and the relationship with the vaginal wall, rectal wall and urethra were carefully evaluation. Then 1.5mL SonoVue were injected into the median cubital vein. According to the CEUS images, the lesion enhanced time, enhanced level and enhanced morphology were recorded. The three cases were uterine leiomyoma with mucinous degeneration, high-grade squamous intraepithelial lesions (HSIL/VaIN III) with local infiltrating carcinoma and fibroma. The leiomyoma and fibroma were exhibited homogeneous enhancement followed by apparent hypoenhancement. The margin of the tumour regular and complete capsule were depicted clearly. The infiltrating carcinoma was exhibited heterogeneous hyperenhancement, the margin of the tumour irregular and no membraniform sign. Through CEUS, it can also evaluate the microblood flow of the tumour and differentiate benign from malignant. The transrectal dual-planar ultrasound probe can clearly show the lesion's size, boundary, margin, shape, and relationship with the surrounding tissue. Through CEUS, the microblood flow can be evaluated and the benign and malignant tumours can be distinguished, which provides a new diagnostic method for accurate evaluation of vaginal tumours.
