EP14.28: The outcome of twin pregnancies delivered in a nine‐year period

Outcome of twin pregnancies delivered in a nine year period at the Institute for the Care of Mother and Child in Prague. Retrospective analysis of 1576 twin pregnancies delivered at the Institute for Mother and Child Care in 2012 - 2020. A total of 931 dichorionic diamniotic (DC) and 442 monochorionic diamniotic (MC) twins delivered after 24 gestational weeks (w) without genetic or morphological defects or intrauterine intervention were statistically analysed. Monochorionic monoamniotic twins were included in the MC group for statistically insignificant representation (18 cases). The outcome of a subgroup of 203 MC twins who underwent intrauterine surgery (MCS) was also analysed. Pregnancies complicated by the intrauterine death (IUD) of one or both fetuses after 24w were included in the study. Of the total number of 1576 twin pregnancies 931 (59,1%) were DC, 442 (28%) MC, and 201 (12,9%) MCS. The median of the gestational week was 37w, 35w, and 33w in DC, MC, and MCS, respectively. Iatrogenic indication for the delivery, premature rupture of membranes, and spontaneous preterm labour were in 66,3%, 17,3%, and 16,4%, respectively. In total, the majority (91,9%) of pregnancies were delivered by Caesarean section. The mean birthweight of the bigger fetus in the DC, MC, and MCS twins was 2490g and 2070g, 1595g respectively. The median birthweight of the smaller fetus in the DC, MC and MCS was 2200g, 1810g, and 1470g, respectively. The median of the fetal weight difference was lower in double survival cases compared to the group with the survival of only one fetus (10,2% vs. 24,1%; p = 0.000). The study provided a summary of the specific perinatal parameters and their differences in DC and MC twins. MC pregnancies show a higher incidence of perinatal complications associated with higher prematurity and lower birthweight.

Abstract

The study provided a summary of the specific perinatal parameters and their differences in DC and MC twins. MC pregnancies show a higher incidence of perinatal complications associated with higher prematurity and lower birthweight.
