EP05.53: The differential diagnostic value of Doppler technology with highly sensitive flow for fetal intracranial space‐occupying lesions

The aim of this study is to assess the differential diagnostic value of Doppler technology with highly sensitive flow for fetal Intracranial space-occupying lesions. Thirty fetuses with intracranial space-occupying lesions were collected from January 2022 to December 2023, with a total of 39 lesions. Among them, 18 ones were intracranial cysts (including 15 arachnoid cysts and 3 hemispheric cysts), 14 ones were intracranial hemorrhage, and 7 ones were intracranial tumours (including 2 with dural sinus malformations, 1 with Galen venous tumour, 2 with lipomas, 1 with meningioma, and 1 with ependymoma). We retrospectively quantitatively calculated the blood flow index in the area of interest of the above-mentioned lesions by Doppler technology with highly sensitive flow. There was a statistical difference (P = 0.000) among the three groups: intracranial cyst group, intracranial hemorrhage group, and intracranial tumour group. It was found that the differential diagnosis between the cyst group and the tumour group has statistical significance (P = 0.000), and the differential diagnosis between the hemorrhage group and the tumour group has statistical significance (P = 0.000). As the combination of cyst group and hemorrhage was used for differential diagnosis with tumour group, we found the area under the ROC curve was 0.991, and the optimal critical value of the blood flow index for differential diagnosis was 2.1%, with the sensitivity 100% and the specificity 94%. Doppler technology with highly sensitive flow has play an important role for distinguishing intracranial tumours from hemorrhage or cysts. When the blood flow index is more than 2.1%, the diagnosis tends to be intracranial tumours; When the blood flow index is less than 2.1%, the diagnosis tends to be intracranial cysts or hemorrhage. However, the blood flow index is not meaningful for the differential diagnosis of hemorrhage and cysts, and needs to be evaluated in conjunction with grayscale ultrasound.

Abstract

The aim of this study is to assess the differential diagnostic value of Doppler technology with highly sensitive flow for fetal Intracranial space-occupying lesions. Thirty fetuses with intracranial space-occupying lesions were collected from January 2022 to December 2023, with a total of 39 lesions.
