EP16.16: The value of crystal imaging in the diagnosis of placenta accreta disease

The purpose of this article is to examine the value of crystal imaging for the diagnosis of placenta accreta disease. We recruited 25 pregnant women (31.65 ± 3.88 years, weeks) with a history of Caesarean section or uterine cavity operation. Samsung HERA W10 colour Doppler ultrasound diagnostic instrument and Crystal Vue and Realistic Vue software (Samsung Medison, Republic of Korea), abdominal volume probe, probe frequency 4-8MHz was used. Placenta accreta disease was defined as 0, no accreta disease, and 1, placenta accreta spectrum (PAS). Positive crystalline imaging was defined as imaging disruption of the underlying membrane of the placenta. Negative crystal imaging was defined as continuity of the placental underlying membrane. Thirteen placenta accreta events occurred. The number of positive cases of crystal imaging in the event group was higher than that in the no-event group (77% vs 8.3%). Univariate analysis showed that Crystal Vue Flow (+), placenta previa, zona, and subplacental vascular echo disruption were associated with these PAS. Multivariable analysis showed that Crystal Vue Flow (+) was an independent risk indicator for the diagnosis of placenta accreta disease (OR (95% CI) = 30 ((2.63, 342.73)); p = 0.006) receiver operating characteristic curve analysis showed, the best sign to predict the event was the interruption of the membranous sonographic image of the placental underlay in crystal imaging (sensitivity 76.92%, specificity 90%, area under the curve 0.835). Patients with PAS have a higher probability of having an incomplete placental rim on crystalline imaging than those without PAS. Adding crystalline imaging of an incomplete placental rim to routine ultrasound examination of placenta accreta can provide better diagnostic information. Abstract
