EP21.31: Is previous Caesarean section a risk factor for subsequent adnexal ectopic pregnancy?

Adnexal ectopic pregnancy (EP) is one of the leading causes of maternal problems in the world. The present study will aim to investigate the effect of previous type of delivery. The present study was descriptive-analytical and retrospective. The current community and research sample includes all patients with adnexal ectopic pregnancy who have been admitted to Imam Ahvaz hospital for the past year (September 2022-September 2023) and have been treated medically or surgically. All patients were studied as a census. Age, number of births, risk factors (history of surgery on the fallopian tube, previous ectopic pregnancy, pregnancy with IUD, infertility, Caesarean, assisted reproductive treatment with IUI, and information on ultrasound (to confirm the presence of the adnexal lumps) were obtained from the clinical file of patients. The present study was conducted on 180 patients. The average age of patients was 31 years. 55.1 percent had history of Caesarean section and 44.9 percent had history of normal vaginal delivery. 48.8% had no history of miscarriage and 40.8% had a one-time history of miscarriage. 66.2% had no history of ectopic pregnancy, and 30.8% had one and 3.1% had two histories of ectopic pregnancy. 28.3% were reported on their ultrasound of pelvic free fluid. 97.8% had adnexal mass according to ultrasound, but only 2.2% had no reported adnexa mass in their ultrasound. 60.6% of patients were managed medically. There was no significant correlation between the adnexal EP and previous type of delivery. This study showed that type of previous delivery, Caesarean or vaginal delivery was not significantly different in later occurrence of adnexal EP. History of Caesarean section was not a risk factor for later adnexal EP in this study.
