EP09.103: Validation and follow‐up of fetal echocardiography results in early pregnancy

The purpose is to evaluate the cumulative detection rate of fetal CHD in different pregnancy and neonatal periods, focusing on the results of echocardiography in early pregnancy. We conducted a retrospective study of all completed early pregnancy fetal cardiac examinations with validated results from 2019 to 2022. According to the results of cardiac ultrasound examination in early pregnancy, the cases were divided into three groups: cardiac abnormalities (group A), small cardiac lesions (group B), and normal heart (group C). Fetal echocardiography is necessary in the second trimester of pregnancy, and CHD screening needs to be completed after birth. If the fetus is induced to labour, pathological examination is required. A total of 12,139 cases were included, including 148 cases in group A, 96 cases in group B and 11,895 cases in group C. A total of 234 cases of cardiac abnormalities were detected by pathology and clinical verification, including 144 cases of CHD in group A, 12 cases of CHD in group B and 78 cases of CHD in group C (55 cases in middle pregnancy, 5 cases in late pregnancy and 18 cases in neonatal period). The total detection rate of cardiac abnormalities was 1.85%, and the detection rate in early pregnancy was 58.1%. 76 cases of CHD were missed and misdiagnosed by ultrasound in early pregnancy. This study found that 29 cases of CHD progressed with time, including 15 cases of valvular disease, 11 cases of large artery obstructive disease, 2 cases of cardiac tumour and 1 case of cardiomyopathy. Ultrasound in early pregnancy has good diagnostic efficacy, but the importance of sequential ultrasound examination should be emphasised.
