EP25.03: The role of transabdominal 3D ultrasound in the diagnostic of congenital malformations of the reproductive system in pediatric and adolescent patients

Uterine anomalies in adolescent patients may cause problems such as amenorrhea, pelvic pain, painful menstruation, and heavy menstrual bleeding. In subsequent stages of the patient's life, these may lead to infertility, recurrent miscarriage, as well as pregnancy and labour complications. The aim of this study is to present the clinical use of transabdominal three-dimensional ultrasound in the diagnosis of congenital uterine malformation in children. The authors discuss when a congenital defect of the reproductive system in children may be suspected, what clinical symptoms may suggest the occurrence of a congenital defect of the uterus in pediatric population, and which genetic syndromes are correlated with congenital defects of the reproductive system. The authors present the results of 2D and 3D TA USG of several patients referred to the Pediatric and Adolescent Gynecology Clinic due to various gynecological disorders occurring in the population of patients under 18 years of age. A congenital defect was suspected or confirmed in the 3D TA ultrasound, which made planning further diagnostics possible. In case of patients leaving the clinic at the age of 18, the indications for further diagnosis (such as 3D TV US) and the defect's possible impact on patients' fertility were discussed. Basing on a review of the literature and our results, we conclude that 3D TA USG may be an important element in the diagnosis of congenital defects of the reproductive system in children and may become an alternative method to MRI. It may conveniently become the important step in the assessment of the genitourinary tract in pediatric and adolescent patients with a history of primary amenorrhea, pelvic pain or painful menstruation.
