EP06.51: Umbilical artery aneurysm: a hidden treasure with normal outcome

A 32-year-old G1 female presented for her routine prenatal ultrasound examination at 28 weeks of gestation. 2D ultrasound revealed a cystic structure measuring 3.2x1.6 cm involving umbilical artery seen approximately 6.2 cm from the site of fetal cord insertion. Colour Doppler spontaneous flow within the aneurysmal segment of the umbilical artery without evidence of thrombosis (see figure 1a,b). Spectral Doppler showed arterial flow within it (see figure 1c). Power Doppler showed prominence of one of the umbilical arteries prior to its aneurysmal dilatation with associated cord edema (see figure 2). Based on 2D and colour Doppler the diagnosis of UAA was given. The patient was counselled regarding the potential risks. Serial ultrasound examinations were scheduled every two weeks to monitor fetal growth and status of the aneurysm. On follow-up the fetus showed satisfactory growth and stable size of UAA without complications. At 33-34 weeks the UAA showed no flow on Doppler suggestive of thrombus and a two vessel cord was seen (see figure 3a,b). Following this the patient was hospitalised for surveillance. Serial ultrasounds showed normal fetal growth and Doppler indices. Elective Caesarean section was done at 36 weeks. A female baby of 2.6kg was born. The neonate underwent a thorough postnatal evaluation. The infant was discharged on day 2 of life, and follow-up visits at 1 week and 1 month showed normal growth and development. This case highlights the importance of early prenatal detection, careful monitoring, and multidisciplinary management in achieving a successful outcome in cases of UAA. Abstract
