EP09.80: Fetal left isomerism associated with ventricular inversion and normal ventriculoarterial connection

Atrioventricular discordance and ventriculoarterial concordance are one of the rare forms of heart defects. We report a case of fetal ventricular inversion and normal ventriculoarterial connection associated with complex cardiac defects diagnosed at 22.1 weeks gestation. A 29 years old woman (non-consanguineous marriage, G2P1) was referred for the second trimester ultrasound. The first baby boy was born with congenitally corrected transposition of the great arteries- CCTGA. He is 2.5 years old now and doing well. This is the second pregnancy and fetal cardiac examination revealed: Left atrial isomerism; AVSD; PRUV; Left-sided morphologic lt.atrium connected to a left sided morphologic RT ventricle, right-sided morphologic lt.atrium connected to a right-sided morphologic LT ventricle; VA connection was normal- the Aorta arising from the rt-sided morphologic LV and the PA arising from the left-sided morphologic RV; The great vessels are crisscrossing and going to the trachea's right side, making V-sign on the right side. Family decided to continue the pregnancy. A repeat fetal echocardiography at 30weeks gestation confirmed these findings. The baby was born at 38 weeks, on the 6th of March, 2023 with the weight of 3160g and with the height of 49cm. There were no signs of cyanosis. Unfortunately, the baby lived only for 3 days and died on the 9th of March, 2023. The special feature of this case is the recurrence of ventricular inversion in the same family.
