EP09.66: Application of fetal intelligent navigation echocardiography in the diagnosis of tetralogy of Fallot

To investigate the performance of fetal intelligent navigation echocardiography (FINE) applied to spatiotemporal image correlation (STIC) volume datasets in generating and displaying three specific abnormal fetal echocardiographic views, namely, the 3-vessel trachea (3VT) view, left ventricular outflow tract (LVOT) view and right ventricular outflow tract (RVOT) view, in fetuses with tetralogy of Fallot (TOF). Seventy-six fetuses with TOF were enrolled in this prospective study. One or more STIC volume datasets from the 4-chamber view as the initial view for each fetus were collected. One optimal volume per case was selected for online analysis applying the FINE technology combined with virtual intelligent sonographer assistance (VIS-Assistanceâ) to optimise the diagnostic view image. Finally, the visualization rates of three specific abnormal echocardiographic views and six key diagnostic elements were calculated. The reliability and repeatability were analysed. The STIC volumes (n = 68 total) were obtained in 76 patients. The statistical results of three specific fetal abnormal echocardiographic views were 89.7% (n = 61, 3VT), 85.3% (n = 58, LVOT), and 88.2% (n = 60, RVOT). The result of the intraobserver evaluation showed the following: 3VT, Element 1, Element 2, Element 5, Kappa = 0.841~0.925, with excellent consistency; other views and elements, Kappa = 0.745~0.766, with good consistency. The result of interobserver: Element 3, Kappa = 0.817, with excellent consistency; and the other views and elements, Kappa = 0.635~0.746, with good consistency. FINE technology combined with VIS-Assistanceâ can successfully generate and display three specific abnormal echocardiographic views and six key diagnostic elements in fetuses with TOF, with perfect repeatability and consistency. FINE technology can be applied to screening and remote consultation for fetuses with TOF.
