OC01.02: *Effect of aspirin on placental biomarkers profile in high‐risk women for pre‐eclampsia

To compare the temporal changes in MAP, UtA-PI, PlGF, and sFlt-1 across gestation between high-risk (HR) women with and without pre-eclampsia (PE) and low-risk (LR) women. This prospective longitudinal study involved 2,007 women with singleton pregnancies who participated in the first trimester screen-and-prevent program for PE at the Prince of Wales Hospital, Hong Kong. The risk of PE was determined using the FMF triple test. HR women were given low-dose aspirin prophylaxis. The participants were followed up at 12-16, 20-24 and 30-37 weeks to measure MAP, UtA-PI, PlGF and sFlt-1. A linear mixed-effects analysis was performed to compare the longitudinal changes of these biomarkers. Our study involved 403 LR women, 1,471 HR women without PE, and 133 HR women with PE. The HR with PE group exhibited the highest levels of MAP, UtA-PI, sFlt-1, and the lowest level of PlGF throughout pregnancy. Compared to the LR group, both HR groups had higher MAP MoM across gestation (0.064 vs 0.030 vs 0.017; p < 0.001). For UtA-PI MoM, both HR groups had higher values than the LR group (0.087 vs 0.022 vs -0.021; p < 0.001). Both HR groups had lower PlGF MoM across gestation than the LR group (0.056 vs 0.047 vs 0.077; p < 0.001). However, there was no significant difference in UtA-PI and PlGF between HR women with and without PE in the first trimester. HR with and without PE had higher sFlt-1 MoM across gestation than the LR group (0.132 vs 0.012 vs -0.029; p < 0.001). However, the difference was primarily observed in the third trimester. Throughout pregnancy, HR women with PE have higher levels of MAP, UtA-PI, sFlt-1, and lower levels of PlGF than unaffected HR and LR women. Notably, the change of sFlt-1 levels is mainly observed in the third trimester. Abstract
