EP01.53: Assessment of sonographic fetal cardiac evaluation capabilities of perinatology subspeciality trainees: a mixed‐methods needs analysis

Fetal cardiac defects are common. Hence, detailed fetal heart examinations by ultrasound are essential to perinatology training. Although recent core curriculum improvement efforts have led to some progress in how trainees are assessed, there are gaps, particularly in assessing fetal heart examination capabilities. We performed a needs analysis on this subject using a mixed-methods approach. Initially, 30 experts from different perinatology departments nationwide completed an online survey that included questions on the current conjuncture, existing deficiencies, and recommendations on the assessment of fetal heart examination capabilities of perinatology trainees. Then, depending on the initial quantitative data, ten experts from various settings were interviewed on the subject using a semi-structured approach. Thematic analysis was used to examine the data to identify common themes and develop recommendations. The following themes emerged: (1) Deficiencies in documentation of trainee progress, (2) need for objective assessment and evaluation, (3) lack of formal written reflection and feedback, (4) requirement for diversified formative assessment methods, (5) mixed opinions on ultrasound simulation, (6) need for more efficient use of portfolio, (7) use of International Society of Ultrasound in Obstetrics and Gynecology guidelines for standardisation, (8) integration of technology such as artificial intelligence into assessment, and (9) emphasis on professionalism and perinatal counselling. Educational gaps in assessing the sonographic fetal heart examination capabilities of perinatology subspecialty trainees have been defined. A formal programmatic approach to assessment is needed within this context.
