EP01.29: Assessment of a capacity building initiative using clinical teaching and simulation on basic obstetric ultrasound proficiency of residents in India

Current postgraduate curriculum in India lacks practical training in ultrasound for residents in obstetrics and gynecology. This study aimed at developing and assessing impact of a capacity building program utilising clinical teaching and simulation to enhance skills, knowledge and attitudes of residents at a hospital in India. A training program comprising 14 modules on basic obstetric ultrasounds was created over 3 months; validated by fetal medicine experts, underwent a pilot study for face validity. Fifteen newly enrolled junior residents without prior ultrasound experience were recruited. A two-day workshop included interactive module sessions and 5 hours of hands-on training using an obstetric simulator. Pre- and post-tests consisting of 50 multiple-choice questions assessed knowledge and attitude, with follow-ups at 1 and 3 months. Skill assessment at 3rd month was conducted using an Objective Structured Practical Examination (OSPE), alongside image submission by residents. There was significant improvement in marks over time, with a mean pretest score of 25 ± 2.6 out of 50 marks, post-test score of 36.93 ± 3.97 and stable scores [34.4 ± 3.42, 34.4 ± 3.38] during follow-up reviews (p<.0001). OSPE scores at 3rd month averaged at 17.87 ± 3.62 out of 30 marks; image assessment scores averaged at 14.73 ± 1.58 out of 20 marks. Participant feedback indicated a positive response to all program aspects [practicality, content and structured training]. Confidence levels in performing clinical ultrasound procedures on live patients improved significantly after attending the module. Majority (60%) felt confident, while 40% indicated they were slightly confident in performing basic obstetric ultrasound. This program effectively enhanced knowledge, attitude and skills of residents, fostering confidence in performing basic obstetric ultrasounds during their residency.
