EP06.44: Comparing ultrasound and magnetic resonance imaging in predicting perinatal mortality among fetuses with left‐sided congenital diaphragmatic hernia

Prenatal risk stratification remains a challenge in congenital diaphragmatic hernia (CDH). We sought to evaluate the association of different ultrasound and magnetic resonance imaging (MRI) predictors of neonatal mortality in fetuses with CDH. The stomach was evaluated in 62 cases, % liver herniation in 61 cases, TLV in 57 cases, IP-PI in 46 cases, and O/E LHR in 95 cases. Predictors of perinatal mortality are seen in table 1. Prevalence of perinatal mortality was similar among the different indicators. The highest association was found for stomach grade 3 (OR 13 [2.59-65.2]) and total lung volume (OR 8[1.5-41.6]). The O/E LHR performed similar to the Doppler ultrasound, and liver herniation, but lower than total lung volume and stomach herniation as shown in table 1. There were 28/95 (29.5%) perinatal deaths. Prenatal risk stratification remain a challenge in predicting postnatal outcomes, our findings identify that MRI parameters and herniation of the stomach into the thorax may perform better than O/E LHR in identifying neonates at higher risk of perinatal mortality.
