EP17.24: Maternal ophthalmic artery Doppler in normal pregnancy and pre‐eclampsia and its association with pregnancy outcome

To study maternal ophthalmic artery Doppler (OAD) in normotensive and pre-eclamptic women at term and examine the association of maternal OAD measures with pregnancy outcomes in pre-eclampsia. Women with pre-eclampsia were recruited as cases and normotensive pregnant women at term were recruited as controls. Maternal ophthalmic artery Doppler was performed on all. Waveforms were obtained in sequence from the left eye. We recorded the average of the four measurements, for the following indices: first peak of systolic velocity; second peak of systolic velocity; and the ratio of the second to first peak of systolic velocity (peak ratio). The mean values of peak ratio were as follows: 0.53 ± 0.12 in normotensive women; 0.86 ± 0.14 in pre-eclamptic women which was statistically significant. ROC curve for Doppler peak patio as a predictor of adverse maternal outcome in pre-eclamptic patients was plotted. Area under curve was found to be 0.705 for prediction of adverse maternal outcome at a cut-off value of peak ratio ≥0.815. Adverse maternal outcome was observed in significantly higher proportion of women with peak ratio ≥0.815 as compared to lower peak ratio. On statistical analysis, higher peak ratio (≥0.851) had significant association with the following maternal outcomes - hypertensive crisis, PRES (Eclampsia/ CNS injury), HELLP Syndrome. Maternal OAD may prove to be useful point of care tool for the individualization of risk for neurological events in PE, potentially allowing for more appropriately targeted therapy and timely referral, thus ensuing an adequate cerebrovascular response in those identified high risk.
