OP10.07: Novel “shift of shunt” sign to identify ductus venosus systemic shunt at 11–14 weeks: a single‐centre prospective study

Abnormalities in Ductus Venosus Systemic Shunts (DVSS) have been associated with adverse pregnancy outcomes. The aim of the study was early detection of DVSS at 11–14 weeks and introduce this novel “Shift of Shunt” sign as marker in observing the outcome in these fetuses. This was a prospective study conducted at our institution from Dec 2020 to Dec 2022 which included 1000 patients undergoing FTS. In normal fetuses particular attention was paid to the vertical course of umbilical vein and DV insertion into IVC on transverse section of abdomen on colour Doppler (see figure 1). In DVSS, the “Shift of Shunt” sign was defined as deviation of vertical course of umbilical vein and insertion of DV (see figure 2). All patients showing deviation from the vertical course (DVSS) were offered direct testing. 12 of 1000 patients had spontaneous abortions. 8 lost to follow up. Remaining (n = 980) reviewed for “Shift of Shunt Sign”. Normal alignment of umbilical vein and DV insertion into IVC was seen in 958 (97.76%) of 980 cases, and deviation of alignment of UV and DV insertion into IVC (“Shift of Shunt Sign”) observed in 22 (2.2%) cases who were offered direct testing. 2 (9.09%) of 22 cases had Trisomy 21 opted for MTP. Rest (n = 20) had normal karyotype. Follow up of 5 (25%) of 20 with normal karyotype developed early onset IUGR. 7 (35%) of 20 had late onset pre-eclampsia and 8 (40%) of 20 had normal outcome. Strong correlation seen between “Shift of shunt sign” which was found to be statistically significant (p < 0.01). Identification of DVSS at 11–14 weeks using “Shift of Shunt” sign offers valuable information optimising fetal outcomes. Abstract

Strong correlation seen between “Shift of shunt sign” which was found to be statistically significant (p < 0.01). Identification of DVSS at 11–14 weeks using “Shift of Shunt” sign offers valuable information optimising fetal outcomes.
