EP09.84: Ventricular disproportion in prenatal diagnosis of coarctation of the aorta: impact of ventricular deptal defect on diagnosis
Prenatal diagnosis of coarctation of the aorta (CoAo) is challenging, often raised from intrauterine ventricular disproportion but only confirmed postnatally. We aimed to investigate if association with a ventricular septal defect (VSD) weakens predictive criteria. We retrospectively analysed fetal echocardiographic data in pregnancies complicated by suspected CoAo from January 2019 to October 2023, indexing data according to validated z-scores and dividing patients into VSD+ (undergoing corrective surgery in the first year of life) and VSD-. We examined 129 fetuses suspected of CoAo, with 26.4% VSD+ and 73.6% VSD-. Subaortic defects were present in 52.9%, and muscular defects in 20.6%. Table 1 presents indexed valve measurements and ratios compared between the two subgroups, with notable differences in the size of the aortic isthmus. Neonates with VSD had a significantly lower false positive rate, as depicted in figure 1. Our data suggest that prenatal diagnosis of VSD+ CoAo has significantly fewer false positives. Additionally, VSD is associated with greater isthmic hypoplasia, likely aiding prenatal diagnosis. VSD + (n = 34) mean ± SD VSD - (n = 95) mean ± SD Please note: The publisher is not responsible for the content or functionality of any supporting information supplied by the authors.
