EP20.17: Follow‐up of uterine arteriovenous malformation

Uterine arteriovenous malformation (AVM) is a rare entity with approximately 100 cases reported in the literature. It represents a mixture of capillary-like vessels characterised by the presence of shunts between the myometrial arteries and veins. It can be congenital or acquired. Diagnosis is usually performed by transvaginal ultrasonography with colour high-definition Doppler flow, showing a characteristic vascular mosaic pattern with high flow rates and low resistance. Angiography is performed to confirm the diagnosis, usually when surgical treatment or therapeutic embolization is required. We present a follow-up of 36-year-old patient who was diagnosed with uterine AVM in the second month of pregnancy, eight years ago. We assume it was acquired, since she had an uneventful Caesarean section one year prior to diagnosis. Transvaginal US detected heterogeneous myometrial anechoic areas on the posterior wall of the uterus to the right, with increased vascularity and multidirectional flow. After the pregnancy was ended due to medical indication, the diagnosis was confirmed with CT angiography. Since the patient was hemodynamically stable without symptoms, only further follow-up was advised. Currently, she is without symptoms, with the US finding which may indicate spontaneous resolution. Timely diagnosis of this entity is of utmost importance since it presents with non-specific symptoms and US image that resembles gestational trophoblastic disease.
