OC17.02: Adherence to guideline‐recommended aspirin for pre‐eclampsia prevention in Denmark

To assess physician adherence to guidelines regarding aspirin to women at high risk for pre-eclampsia and investigate the association between prescription and specific high-risk factors. This retrospective cohort study was conducted among pregnant women enrolled in the prospective multicentre study to evaluate pre-eclampsia screening in Denmark (PRESIDE). According to national Danish guidelines women with previous pre-eclampsia, diabetes, chronic kidney disease, chronic hypertension, or autoimmune disease are eligible for aspirin prophylaxis. Aspirin treatment was registered for all women in the PRESIDE study when attending the second trimester anomaly scan. PRESIDE data did not include information on chronic kidney disease. Data from 8,155 women were analysed in the PRESIDE study. Among these, 278 (3.4%) were identified as having one or more high risk factors. 114 (41%) of the women who were high risk and therefore eligible for prophylactic low-dose aspirin treatment were prescribed aspirin. Most likely to receive indicated aspirin treatment were those with autoimmune disease, whereas women with previous pre-eclampsia were least likely to receive aspirin (Barchart). Of the 167 individuals who missed the chance to receive aspirin, 136 women had a history of pre-eclampsia (81.4%). The percentage of high-risk women who are correctly prescribed aspirin using the current screening approach is relatively low. Women with previous pre-eclampsia were less likely to receive indicated aspirin treatment compared to women who had other high-risk factors.
