Conference Abstracts
Background: Novel Chemotherapies pose unknown implications for surgery/healing.Dental trauma management is time-sensitive to achieve optimal outcomes, and the potential impacts of novel chemotherapies need to be balanced in the treatment planning process.Presenting Problem: A 10 yr old, with a pilocytic astrocytoma undergoing novel chemotherapy with a Pan-Raf Inhibitor, presented to the dental team at the Royal Manchester Children's Hospital, suffering a severe complicated crown-root fracture UL1 after falling from a fairground ride.Clinical Management: Our agreed approach involved decoronation, root filling with Total Fill bioceramic putty, and root burial under general anaesthesia.A partial denture enhanced with opacities was made to match hypomineralised neighbouring teeth. This provided an optimal aesthetic solution to support our patient's starring role on Britain's Got Talent.Discussion: Management of this complex dental trauma presented several unique challenges to check and balance.Planning required liaising with her oncology and anaesthetic team to ensure patient safety, including pausing chemotherapy. The severity of the injury, the patient's anxiety, combined with time-sensitive aesthetic expectations around appearing on TV from both patient and parents, meant treatment planning required a careful balance to restore aesthetics and function in the short term, while preserving hard tissue long-term.This case highlights the importance of close communication with patient's medical teams to provide dental care in a way that minimises any impact on ongoing medical treatment.Novel drug classes present unique challenges as they become more prevalent and their potential influence for paediatric dentistry is as yet undefined.
