O06 Combined MIVI-Q aspiration catheter-stent retriever approach to primary medium vessel occlusions stroke thrombectomy: preliminary clinical experience

<h3>Introduction</h3> Emerging data suggests the safety and effectiveness of EVT for primary medium vessel occlusions (pMeVOs) like distal M2 and the M3 segments as well as the ACA and PCA, in part due to the improvement of stent-retriever (SR), aspiration catheters and endovascular techniques. <h3>Aim of Study</h3> We assessed the efficacy and safety of the novel MIVI QTM aspiration catheters in combination with SR devices in the treatment of pMeVOs. <h3>Methods</h3> We retrospectively analyzed demographics, procedure characteristics, and clinical data from consecutive patients with acute pMeVOs treated with a combined approach using MIVI QTM aspiration catheters and SR. Reperfusion was defined according to the Modified Thrombosis in Cerebral Infarction (mTICI) score. Clinical outcomes were measured by the National Institute of Health Stroke Scale (NIHSS) and modified Rankin Scale (mRS) scores. <h3>Results</h3> We included 40 patients [median age, 74 y (IQR:62-80); 24 (60%) male; 34 (85%) with occlusion in the M2/M3 segment of the MCA, 5 (12.5%) in the PCA, and 1 (2.5%) in the ACA; median NIHSS score at admission was 12.5 (IQR:9.25-18)]. After the first pass, 19 (47.5%) patients had mTICI≥2c. At the end of the procedure, 39 (97.5%) had mTICI≥2b and 31 (77.5%) had mTICI≥2c. No serious device-related adverse events were observed. Symptomatic intracranial hemorrhage developed in 4 (10%) patients. At 90 days, 21 (52.5%) patients were functionally independent (mRS 0-2). <h3>Conclusion</h3> This preliminary study found good efficacy and safety for MIVI QTM aspiration catheters used in combination with SR devices in the treatment of pMeVOs. <h3>Disclosure of Interest</h3> no.
