Reimagining Reason’s Swiss Cheese Accident Causation Model
More Than 30 years ago, psychologist James Reason developed a model to illustrate how major accidents and catastrophic systems’ failures tend to be based on multiple, often seemingly trivial failures <xref ref-type="bibr" rid="ref1" xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink">[1]</xref> . The model depicts the accident as a series of unfortunate events in both human and component failures.
