54032 Online patient attitudes towards atopic dermatitis treatment through the eras of dupilumab and Janus kinase inhibitors: a sentiment analysis of Reddit
Background: The landscape of atopic dermatitis (AD) treatment has undergone two major shifts with FDA approval of the following medications: dupilumab on March 28, 2017, and oral Janus kinase inhibitors (Jakis), upadacitinib and abrocitinib, on Jan 14, 2022.To our knowledge, no studies have been done to evaluate how patient satisfaction and attitudes towards management of AD have changed throughout the three ''eras'' of treatment. Methods: Data were collected, using Python and Reddit's Application Programming Interface (API), from the 1,000 "top'' and 1,000 most "controversial'' posts on the /r/eczema subreddit.Posts and comments were categorized as "pre-dupilumab,'' "post-dupilumab,'' or "post-Jaki,'' and sorted by post date.Attitudes were measured through two natural language techniques: lexicon-based analysis of individual words (Vader), and transformer-based methods which capture wider sentence context (RoBERTa).Two-tailed Mann-Whitney U tests evaluated relationships between treatment terms and treatment eras by comparing average lexicon-and transformer-based sentiment scores. Results: The inclusion of ''post-dupilumab'' and ''post-Jaki'' terms were associated with statistically significant increases in sentiment, both in lexicon-based (Vader) and transformer-based (RoBERTa) sentiment analysis methods (post-dupilumab: Vader p\0.001,RoBERTa p\0.023; post-Jaki: Vader p\0.001,RoBERTa p\0.001).''Pre-dupilumab'' terms were not statistically significant, likely related to a small number of posts (n21).Conclusion: Inclusion of ''post-dupilumab'' and ''post-Jaki'' terms were associated with statistically significant increases in sentiment with both types of sentiment analysis.Future monitoring of sentiment shifts could provide valuable insights into public perception and acceptance of future therapeutic approaches.
