Obturator hernia should be included in the differential diagnosis of intestinal obstruction of unknown origin, especially in elderly women with low BMI. Early surgical intervention is imperative to avoid intestinal resection and reduce complications. Laparoscopy has a tendency to rapid recovery but has its limits.
