Overcoming Resistance
Melanoma, a leading cause of skin cancer deaths globally, has an incidence rate of 2.7 per 100,000 people. In South Africa, it is the fifth most common cancer among males and the sixth among females. Melanoma development is linked to genetic abnormalities that enhance cell survival mechanisms. While early-stage melanoma can often be treated surgically, metastatic melanoma presents significant treatment challenges due to complex molecular pathways and potential acquired resistance to targeted therapies. The relationship between the gut microbiome and therapeutic responses in melanoma adds further complexity, necessitating careful management to prevent resistance and adverse effects. To combat these challenges, combination therapies targeting multiple molecular pathways have emerged as a promising approach. Research is increasingly focused on the tumor microenvironment (TME) and its role in melanoma resistance, leading to innovative strategies like blocking the histone demethylase PHF8 to bypass resistance. Additionally, radiation therapy combined with immunotherapy is being refined to overcome resistance. An exciting new development in melanoma treatment is the creation of nano-enhanced cancer vaccines, offering a novel approach to combating this deadly disease.
