Past the edge of chaos
This chapter examines how complexity and poor system design exacerbate risks during problem escalation. Using Helios Airways Flight 522 as a case study, it shows that system brittleness and misaligned designs hinder adaptive responses. Safety emerges from interactions within systems, requiring resilience-focused designs. The chapter advocates scenario-based design and alignment between risk models and operational realities to handle unpredictability.
